Abstract

Bhakti Husada Krikilan Banyuwangi Hospital has a work accident incident form. But on the work accident form there are items that are still not listed. This study aims to create a work accident incidence form in support of the 2017 edition of the National Accreditation Hospital Standard at the Bhakti Husada Krikilan Banyuwangi General Hospital. This type of research is qualitative. Data collection is done by observation and interview. The theory used in this research is Huffman (1999) which covers physical aspects, anatomical aspects and content aspects. Based on the physical aspects of the form using 80 gram HVS paper, portrait form, using black ink and white colored paper. Based on the anatomical aspects of the form the heading is located at the top, there are instructions, use the Times New Roman 12 font and the form filling is done manually. Based on aspects of the contents of the form the items are complete, but still need to add items, there are no groupings, the items are arranged in order and there are no terms, abbreviations and symbols. Based on the identification of user needs related to items on the work accident form there are additional items for the victim's name, address, employee status, event analysis, equipment / property and toxic and hazardous materials and authentication. Hospitals should report work accidents based on the design of work accident forms that have been made by researchers. the researchers then evaluated the implementation of the work accident incident form at RSU Bhakti Husada Krikilan Banyuwangi. Make a supporting form in supporting the value of National Accreditation Hospital standards at Bhakti Husada Krikilan Banyuwang public hospitals such as patient safety and infection surveillance
