Abstract

Candipuro Community Health Center is the closest primary health facility that handles victims of the Mount Semeru eruption in the Candipuro sub-district and surrounding areas. Patient handling during the Semeru eruption disaster and other disasters at the Candipuro Community Health Center still used emergency medical records so that they could not accommodate detailed information on patients who were victims of natural disasters in accordance with government regulations. This has an impact on the overall recording needs of disaster patients which cannot be met according to standards and recording is not effective and efficient, resulting in non-uniformity of data in medical records. This research aims to analyze the design of medical records and prepare technical instructions for filling out disaster forms at the Candipuro Community Health Center, Lumajang Regency. This type of research is qualitative, with data collection techniques including; observation, interviews, and documentation. The research subjects consisted of 1 head of the community health center, 1 village nurse, 1 emergency room nurse, and 1 medical records officer. The design of natural disaster medical records is in accordance with those produced through data analysis using the theory from Huffman (1999), namely physical aspects, anatomical aspects and content aspects. The final result of this research is a natural disaster medical record design that complies with effective and efficient standards as well as technical instructions for filling out medical records according to user needs.
