Abstract
Completeness of informed consent form filling is very important because it may affect the quality of medical records and health services. Based on preliminary studies, the filling of inpatient informed consent forms at BaliMed State General Hospital Bali was still not completely filled in January-July 2022 with the average incompleteness was 20.83%. Incomplete filling of informed consent might result in weakness of legal evidence and make it not in accordance with the use value of medical records. This study aimed to analyze the factors that cause incomplete filling of inpatient informed consent forms at BaliMed State General Hospital Bali. This type of research was qualitative with data collection techniques of observation, interviews, documentation, CARL, and brainstorming. The subjects of this study were 3 doctors, 2 inpatient nurses, and 1 assembling officer, the head of medical records and human resources. The object of the research was the inpatient informed consent forms. The results showed that the cause of incomplete ability was the lack of knowledge of doctors and nurses, and training had never been held related to filling out informed consent forms. Motivation was the absence of rewards and punishments for doctors and nurses. Opportunity was the absence of standard operating procedures and job descriptions of officers related to the completeness of informed consent forms. The results of the problem priority obtained there were no standard operating procedures, training, and lack of knowledge of doctors and nurses related to filling out informed consent forms. The efforts to fix the problem are to create, socialize, and give to each poly so that doctors, and nurses are aware of the standard operating procedures that have been made, and send doctors and nurses to attend trainings, socializations, or seminars related to filling out informed consent.
