Abstract

Provision of medical records at the Patrang public health center in Jember District was still not accardance with the time necessary when patiens register to outpatients medical record file received by officers with a standard time ≤ 10 minutes. The research was based on the results of observated by researchers that medical record file was not accordance standard, reaching 83% for outpatient the porpose of this research was to analyze the factors providing outpatient in Jember district. The type of this research used qualitative analysis. Data callection techniques using observation, intervew and braibstorming. The subjacts of this research were head of medical record, registration officer and filling officer. The result showed based on disciplinary factors, officers do not directly search for documents in the filling room and search still waiting for some medical record number to be search and medical record files were not neatly ar ranged in the storage room. a suggestion for evaluating coordination related to the provision of medical record files by involving parties and related sections, needs to be considered about adding SIMPUS (Public Health Center Management Information System) to the registration section and making expedition books on the shelf of medical record file storage. The addition of making SOP (Standard Operating Procedure) provision of medical record file out patient that server form working system and the flow of work regulary.
