Abstract
Medical record defect is the incompleteness of medical records such as torn, faded, and illegible. Medical record defect can be seen from intrinsic and extrinsic factors. The purpose of this study was to analyze the factors causing medical record defect in the filing room of the Kalianget Health Center. This research was conducted qualitatively. Data collection techniques are interviews, observation, documentation and brainstorming. The subjects of the study were filing officers, counter officers, and heads of community health centers. The results of intrinsic factor research in terms of the quality of paper, ink, and adhesives are good. The extrinsic factors in terms of physical aspects are that there is no air conditioning and thermohygrometer, dust on medical records, and and full filing racks so that they are crowded and torn together. The biological aspect is fungi in medical records. The chemical aspect is that medical record ink fades. The negligence aspect is that the medical record was exposed to a drink spill, and the SOP for maintaining medical records was not implemented optimally so that torn medical records were not replaced by officers. The results of USG are fungi, dust in medical records, and there is no air conditioning in the filing room. The results of brainstorming are placing medical records in in the open, regulating air ventilation, making a routine schedule of room cleanliness using a vacuum cleaner, and applying for the procurement of air conditioning and thermohygrometers. Suggestions for puskesmas are adding filing racks, retention, and conducting socialization of medical record maintenance at least once a month.
