Abstract

Codification is a method that is able to uniform data collection of individual diseases and medical procedures in patients in order to ensure accuracy, precision, timeliness and actions that will be used as input for a developed management information system. The accuracy of the diagnostic code and medical procedures is intended so that the information produced is precise and accurate which supports the achievement of good quality hospital services. The impact of inaccurate diagnostic codes and medical procedures affects the cost of health services. The purpose of this study was to analyze the causes of inaccurate diagnostic codes and medical procedures. The research method used is literature review through search by reading various sources of articles and other reference materials according to the research topic. Review as many as 30 articles that meet the criteria for articles and research topics. The results showed the factors that caused the inaccuracy of the diagnostic code and medical procedures based on this review literature were; Man factor, namely coders are less thorough in the process of coding medical diagnoses and procedures;The money factor is the absence of budget for training for coders;(3) Material factor, namely doctors' notes in medical record documents that are difficult to read; Machine factors, namely ICD 10 WHO that have not been updated; The method factor, namely codefication not using ICD, auditing coding not yet available, inaccuracy in selecting the main diagnosis, Standard Operating Procedure (SOP) for diagnostic codefication and medical procedures not yet available.
