Abstract

An informed consent form is an agreement for the procedure to be given to a patient which completeness must reach 100%. A preliminary study found 46 out of 140 informed consent forms were incomplete. The purpose of the study was to analyze the factors causing the incompleteness of informed consent form general surgical patient Baladhika Husada Jember Hospital using the 7M theory (Manpower, machine, methode, material, media, motivation, money). Type of this research was qualitative research with data collection through interviews, observation, documentation, (urgency, seriousness, growth (USG), and discussion. The results showed that on the doctors and nurses did not know the standard of informed consent completeness. Comprehensive training has not been carried out. The methode variable there was no SOP of informed consent filling. The material variable showed there was no checklist of informed consent completeness. The media variable showed no special workplace for doctors. The motivation variable showed no reward for doctors and nurses who have filled the informed consent completely. The priority results of using urgency, seriousness, growth (USG) are that officers did not know about the standards for informed consent completeness. The other there was no reward when the doctors and nurses filled informed consent completely. There was no SOP for filling of informed consent filling. There was no checklist of completeness of informed consent. Training has not been carried out thoroughly for doctors and nurses, and there was no doctor's workspace. The recommendation are training on filling out the informed consent form thoroughly to nurses and doctors, make SOP for informed consent filling forms, make completeness checklists for informed consent filling forms, and give compliment rewards or gifts to doctors or nurses who have filled out the informed consent form completely, using space as a doctor's workspace in providing informed consent forms to patients.
