Abstract

The emergency form at Cermee Community Health Center still lacks a column for diagnosis codes, lacks instructions for filling out the form, does not provide a space to write the full names of the doctors and nurses who provided care to the patients. This leads to an incomplete data assessment process and results in an inaccurate and incomplete identification of patient care needs. As a consequence, the diagnoses did not accurate and comprehensive. The aim is to redesign the emergency medical record form at Cermee Community Health Center in Bondowoso Regency. The research methodology employed is qualitative research, with the research subjects being the head of the health center, nurses, and medical record staff. The technic collectionof data this research interviews, observations, and documentation. The results are, in terms of the physical aspect of the emergency medical record form, indicate that it is printed on 80-gram HVS paper, in F4 portrait size, and is white in color with black ink. In terms of the anatomical aspect, the following modifications were made to the emergency medical record form: the heading includes the acronym "UPTD" for the health center's identity, the date of issuance is included, the page number is placed at the bottom of the form, the introduction section includes the title "Emergency Form", the instruction section provides concise filling instructions, the left margin in the body section is expanded to 2cm, and the close section includes the full name of the doctor. In terms of content, the division is adjusted based on the content of the Emergency Department Form of the Ministry of Health No. 269 of 2008, with adaptations made to the grouping, item sequence, and determining the filling method in the design of the emergency patient form. The researcher's suggestion is that the designed emergency medical record form can be implemented in Cermee Community Health Center, Bondowoso Regency, in accordance with the established needs and standards.
