Abstract
Patient registration services at Wira Sakti Clinic are still carried out manually using books assisted by Microsoft Excel. Data recording in excel files is grouped based on monthly patient visits. This can make it difficult for officers to search for data on patients who have not visited for a long time. As a result, services become hampered and take longer. Bank records, medical record numbers used still rely on notebooks. Manual recording results in duplication of medical record numbers. Therefore, an information system is needed that can solve these problems by creating a registration information system that can display medical record numbers automatically so that there is no duplication of numbers. System design methods using waterfall which include requirements analysis, planning, system modeling, program code generation and system testing. The needs analysis stage is carried out by collecting interview data directly to users. The system modeling phase uses flowcharts, data flow diagrams and entity relationship diagrams. The stage of creating program code using PHP programming language, boostrap and codeigniter. The system testing phase uses the black box testing method which focuses on testing the functionality of a system. The result of this research is an outpatient registration information system that can facilitate the performance of officers and can improve the quality of service.
