Abstract

Hospital services during a disaster, officers are required to continue to document the services that have been provided to patients because of the importance of documentation as a source of data that will later be used by the hospital for various purposes. Based on a preliminary survey at Balung Jember Hospital still does not have a special disaster medical record form, so the form used for disaster patients still uses the emergency installation form. This study aims to design or design disaster medical record forms at Balung Jember Hpspital. The method used is qualitative with observational data collection techniques, interviews, and brainstorming. The results of this study are based on physical aspects, designing disaster medical record forms using white paper with a size of F4 80 grams in the form of portrait orientation. Based on the anatomical aspects of designing disaster medical record forms, the heading includes the logo, agency name, agency address, and agency telephone number, for the introduction has been represented by the title. Based on the aspect of the contents of the disaster medical record form, the data enters data according to the disaster data items, namely the patient's social identity, the social identity of the patient's introduction, the type of disaster, the location of the patient found, the patient's examination, the results of the patient's examination and the closing (authentication). The suggestion from the researcher is that the results of the design of natural disaster medical record forms can be implemented at Balung Jember Hospital.
