Abstract

The application of health service in Jelbuk’s local government clinic is related with medical record. Providing a medical record’s entry note (borrowing and reversion) is the employee of counter’s (registration officer) responsibility in Jelbuk’s local government clinic which is done manually and cause a problem. The problem is borrowing and reversion’s data of medical record was not recorded well so it caused misfile and duplication also retention of medical record could not be done yet. Solution of the problem is to make Designing and Making The Web-based Borrowing, Reversion, and Retention Information System (E-Piket) of Medical Record as the purpose of the research. The E-Piket made with the Research and Development, Rapid Application Development (RAD) type as the system development and documentation method, brainstorming, and also interview as the data collecting technique. The 1st RAD phase of designing E-Piket is requirements planning which analyze the purpose and requirements of the system’s need. The 2nd RAD phase is workshop design (system design) which implement Flowchart System, Context Diagram, Data Flow Diagram with the 1st until the 2nd level, Entity Relationship Diagram and workshop design (system design) into the PHP program’s terms with Framework Codeigniter. The 3rd RAD phase is implementation (system description) which is done to the every user and implementation (system trial) which examine the system’s functionality by the users with 100% of success or without an error. E-Piket helps the officer in controlling the medical record and it needs another development of an integrated information system to support the medical record activity.
