Abstract

The Minister of Health of the Republic of Indonesia Number 129 / Menkes / SK / II / 2008 states that the contents of the medical record document should be written in 100% complete information. Outpatient medical records in Pratama Restu Ibu Clinic, Ambulu showed that based on 75 outpatient care medical records, 44% were incomplete or 33 documents of them were incomplete. Then the clarity of writing in outpatient care medical records reached 15%, namely 11 of 75 medical record documents. The purpose of this research was to design a web-based outpatient care medical record information system in Ambulu Pratama Clinic that can facilitate officers in filling out the medical records of outpatients. It also became an effort to overcome incomplete medical record filling or the weaknesses of other manual medical records. This research was included as qualitative research. A Web-based program designed using the PHP programming language and supported by MySQL databases. The software development method used is the Waterfall Model. The data collection techniques in this study used interview, observation, documentation and brainstorming techniques. The objects in this study were outpatient medical records consisted of general polyforms, poly nutrition, KIA poly and outpatient registration forms while the subjects of this study consisted of 6 people, namely the head of medical records, one doctor, one nurse, one nutritionist, one midwife, and one registration officer at Pratama Restu Ibu Clinic, Ambulu.
