Abstract
Patients come to health services with emergency need immediate care. Documentation health service provided to emergency patient is very important as source of data which will be processed into services information. Unfavorable from design will make medical officer who reads the information incorrectly, slow documentation of the patients condition and handling that is not fast enough. Based on preliminary study at Mojoagung health center, emergency medical record form at Mojoagung health center not according to standart. The purpose of this study is to evaluate emergency medical record from at Mojoagung health center Jombang district. The method of research used is qualitative with observation data collection technique, interviews and brainstorming. The results of this study can be known the design of emergancy medical record form not according to standarts emergancy medical record form not according to standarts and user needs which causes filling incompleteness. Therefore, that the new design of emergency medical record form, by considering 3 important aspects, namely physical aspects, aspects anatomy and aspects of content. Furthermore, it will be evaluated using the 4 stages of action research methods, namely diagnosing action, action planning, action taking and action evaluating so as to produce forms that are in accordance with the standards and needs of users.
