Abstract

Community Health Centres is one of the agencies working in the health field. Panarukan Health Center still uses manual methods for outpatient service systems. If the patient does not bring the KIB the officer has difficulty finding patient data, if the file isn’t found then the officer makes a new file, which means that one patient can have more than one different medical record number resulting in duplication of the file resulting in the continuity of the patient's examination results. The purpose of this study was to design and create a web-based general poly registration and outpatient information system at Panarukan Health Center. The type of research used is qualitative research. Data collection techniques used were observation, interviews, and brainstorming. The results of this study were a web-based general poly registration and outpatient information system at Panarukan Health Center using the waterfall method introduced by Somerville. The system design process uses a flowchart system, context diagram, data flow diagram, entity relationship diagram. Using PHP codeigniter programming language, MySQL. This information system can provide convenience to officers to search patient data, there are access rights, there’s a form of patient data, users, actions, and diseases, there’s a menu of registration, examination, coding, and retention, and can produce reports including patient visit reports, Top 10 disease reports, and retention reports. This system can also produce valid and continuous data.
