Abstract
The informed consent form is a form that contains of the consent given by the patient to the doctor to take medical action after the doctor given an explanation. Written approval is given to high-risk medical measures. Based on a preliminary in 2018 and take place in Mitra Medika Bondowoso District Hospital, the highest operative action in 2017 was sectio caesarea reaching 106 actions. There were often complaints to the hospital because patients experience a risk after sectio caesarea, such as bleeding and suture tear, the occurrence of complaints because the patient does not get information about the procedure sectio caesarea clearly. The design of an informed consent form has not used paper that appropriate with the standard and there is no grouping data about patient's clinical data. This study aims to design an informed consent form in sectio caesarean patients in Mitra Medika Bondowoso District Hospital 2018. The method used is qualitative with the technique of collecting data on observation, interviews, documentation and brainstorming. The results of this study are physical aspects, apects of anatomy and content aspects in the design of the informed consent form that used is not in accordance with the standards and needs of users so that it can cause incomplete filling and the information conveyed to patients is unclear. The Conclusion of this study is there several aspectscis not in accordance with the standards and needs of users so that the new informed consent design is designed taking into three importans aspects, namely physical aspects, apects of anatomy and content aspects in accordance with the standards and needs of users.
