Abstract

The completeness of filling out the operation report form based on the survey showed that 75 surgical case report forms in March - May 2022 were not filled in completely. The purpose of this study was to analyze the factors of incomplete filling of the operation report form in the admissions section of inpatients at RSUD dr. Abdoer Rahem Situbondo with personal, leadership, and system variables. The type of research used is qualitative with data collection techniques of interviews, observation, documentation, ultrasound and brainstorming. Informants in this study consisted of 3 nurses, 1 surgeon, and 1 head of medical records. From the results of the study, it was found that personal factors related to attitudes held by surgeons and nurses were still relatively low, leadership factors, in this case there was no strict punishment, and system factors, namely related to the absence of special SOPs. The priority problem that must be solved first is that there is no special SOP for filling out the operation report form and has never been socialized with a score of 70, the second priority problem is that the surgeon or nurse's level of awareness is still low in filling out the operation report form with a score of 59 and the third priority problem is not there is strict punishment with a score of 39. Suggestions that researchers can give to RSUD dr. Abdoer Rahem Situbondo can give a written sentence from the director, prepare special SOP and carry out socialization evenly to surgeons and nurses.
