Abstract

Medical record retention is generally carried out every 2 years for services at puskesmas. Retention must be carried out to limit the amount of storage so that it does not accumulate. The results of the preliminary study at the Maron Health Center, Probolinggo Regency, showed that there was a delay in the implementation of retention and destruction of medical record files. The purpose of this study was to analyze the factors causing delays in the implementation of retention and destruction of medical record files at the Maron Health Center, Probolinggo Regency with variables of personal factors, leadership factors, team factors, system factors and contextual/situational factors. The type of research used is qualitative with data collection techniques in the form of interviews, observation, documentation and USG. The informants in this study consisted of 1 head of administration, 1 officer in responsible person of UKP, 1 head of medical records and 1 medical records officer. The results showed that the 3 priority causes of the top problems using the USG method were system factors, the unavailability of SOP’s related to medical record destruction. Contextual/situational factors, each officer experiencing double jobs. System factor, the unavailability of budget for medical record retention and destruction activities. The research suggestion is to make a SOP for the destruction of medical records and be socialized to medical record officers.
