Abstract

Duplication of medical record numbers still occurs at Pandanwangi Health Center with 10 medical record numbers from 3 storage shelves being duplicated. This has an impact on writing the patient's medical history, especially in the patient's referral letter, so it takes time to justify while the patient has to wait a long time. This research was to analyze the factors that causative factor of duplication medical record numbers at Pandanwangi Public Health Center Malang City based on Gibson's performance theory. The type of research was qualitative with interview, observation, and documentation. The results of research were based on individual factors was not all officers know about the numbering system, the educational qualifications of officers did not meet the standards, the length of service of the officer was more than two years. Based on organizational factors that was the use of facilities and infrastructure had not been maximized, no evaluation from supervisors, no standard operating procedure for numbering, there was already an organizational structure. Based on psychological factors was rewards or punishments in the form of praise or reprimand, the attitude of the officers is less than optimal. Researchers suggest recommending to Public Health Center to provide training to officers, to upgrade infrastructure, write warning letters to patients to bring their identity cards for treatment when they return for treatment, do an evaluation with superiors, compiling standard numbering operational procedures, giving rewards to officer.
