Abstract

Medical record is a from that contains of information about identity of patients , anamnese , the determination of physical laboratory , the diagnosis of the service and the act of medical exam given to patients and the treatment of both hospitalized for in-patients, outpatient care as well as those which received health emergency . This research uses analytic research with the approach of cross sectional study . statistical test using spearman correlation test with α = 0,05. Based on research results obtained more that half of implementation results document medical record by the incomplete (93,6). The resuls showed that is a significancy between knowledge and attitude with the document medical record in hospital kaliwates jember with the correlation ρ =0,030 to the knowledge by the document medical record and ρ = 0,046 to relations with the attitude charging documen medical record. Let the hospital training and related seminars documentation medical record, for good understanding will have an influence to attitude and knowledge health worker at hospitals kaliwates jember
