Abstract

Complete informed consent can be used as evidence in legal cases and can be used as a tool for analysis and evaluation of the quality of services provided by clinics or hospitals. Based on the observation results of the 30 informed consent sheets, the average completeness rate for medical action approval (informed consent) was 77% and 23% incomplete. The number of components has not been filled so that the percentage of completeness does not reach 100 and does not reach the standard. The purpose of this study was to analyze the incompleteness of filling out informed consent at Dr. M. Suhermaan Jember. This type of research is descriptive qualitative research with data collection by observation, in-depth interviews, documentation, and brainstorming. The subjects of this study consisted of 2 medical record officers, 2 emergency room nurses, and 1 general practitioner. The research object was 96 informed consent sheets using simple random sampling. Data credibility using triangulation of sources and techniques. The results obtained at the time of observation of 96 outpatient informed consent sheets in August – November 2022 found incomplete filling of informed consent by 35%. From the results of the analysis, it was found that incomplete informed consent was due to the staff's lack of knowledge about the components that had to be filled out, the officer had never conducted training, and the officer's lack of understanding of the existing SOP. Suggestions that can be given are training proposals for officers implementing informed consent and re-socialization of SOPs and filling guides.
