Abstract
Medical record file retention activities that have been carried out at UPT. Poliklinik Politeknik Negeri Jember has problems with manual file sorting which begins with the officer checking the month and year of the patient's last visit for treatment at the SIM Klinik owned by the Polyclinic, then taking several medical record files on the filing shelf and recording the medical record number for sorting. Meanwhile, the files stored on the storage shelf with a large amount of them overwhelmed the officers and had to sort out the files one by one. The type of research used is Research and Development with waterfall system development methods, there are Requirement Definition, System and Software Design, Implementation and Unit Testing, Integration and System Testing. The data collection technique uses interviews and observations with 2 (two) respondents, namely the head of the polyclinic and the filing officer. This research aims to design and create a web-based medical record retention information system by minimizing if there are errors in sorting medical record files and facilitating the performance of medical record officers in the filing section in sorting out medical record files to be retention so that they match the list of medical record files that will be generated in this information system. The results of this research are a web-based medical record retention information system created using the PHP programming language.
