Abstract
Nosocomial infections and incidents of patient safety if not reported will affect the quality of service and the satisfaction of the patient. At baladhika husada jember hospital, an issue was found in the absence of incident forms for patient safety and infection surveillance. This form is important as evidence in setting standard accreditation because accreditation requires written document evidence. This research aims to design patient safety incident forms and infection surveillance forms based on a snars issue 1 2017. The kind of research used in this study is research & development to design patient safety incident forms and surveillance infections. Data-collection techniques by using interview, observation, and brainstorming techniques. The subjects for this study are medical records, doctors, nurses, ppi and snars accredited teams. The study resulted in a physical aspect identification of the paper used as f4 paper weighing 70 grams and white in weight. The aspect of anatomy is the aspect of the anatomy used on the form called the head of the form (heading) lies at the top, but there is an override on the form, a font used which is arial at 1.5 spaces, and the way the application is handled manually. The content aspects represent an item completeness, item sequencing, item grouping, abbreviations, symbols and ways of filling have been adjusted to other forms and user needs. This study results are the expected design for consideration materials in the scheme of a form in particular a patient safety incident and infection survey. Making it easier for hospitals to do record-keeping and reporting activities about the patient safety incidents.
