Abstract

The general limitation of medical record files being declared inactive in non-hospital health care facilities is 2 years calculated from the date the patient is treated. At the Siliragung Health Center, Banyuwangi Regency, data was obtained that the health center still had not carried out the retention and destruction process so it was said that it was not in accordance with the standards and regulations for shrinking and destroying medical record documents. The purpose of this study was to analyze the factors causing the retention and destruction of medical record files. The method used in this study is USG (Urgency, Seriousness, Growth) to determine the priority of the problem. The type of this research is qualitative research with data collection techniques using interviews, observation, documentation and FGD (Focus Group Discussion). The results of this study were obtained 3 priority causes of the top problems, namely there was no SOP related to retention and destruction with a score of 35, officers had never attended training on retention and destruction with a score of 32, the last educational background of officers did not match the qualifications of medical records with a score of 31. So it is hoped that with this research the Siliragung Public Health Center in Banyuwangi Regency when recruiting at least a D3 Medical Record and Health Information education, it is better for the puskesmas to hold training on retention and destruction of medical record files so that officers better understand the implementation of retention and destruction of medical record files
