Abstract

Clinics are health service facilities providing basic medical services. According to a preliminary study conducted at the Kimia Farma Primary Clinic, Jember, there was incompatible retention that had not been in the destruction of medical records. This study aims to determine the factors causing incompatibility of retention, and the destruction of medical records that the Kimia Farma Primary Clinic Jember had not carried out. This type of research is qualitative, using 7M management data analysis and the USG method with 1 medical recorder, 1 head of clinics, and 2 nurses as research subjects. The results indicated several lacks, for example, human resources for medical recorders, money, machine, material, method, and media issues. There is no proposal to provide the equipment for annual retention and destruction activities. There are also No. scanners and destroyers. Also, there are no shelves and rooms for storing inactive medical records. In addition, the socialization of SOP and the implementation of retention by SOP have not been carried out yet. There is also no unavailability of a retention schedule and destruction of medical records. The researcher advises the medical recorder to open recruitment for medical recorders, make a proposal for budgeting the tools and other supporting facilities for retention and destruction, do socialization about SOP retention and destruction of medical record, and do retention and destruction at the Kimia Farma Clinic Pratama Jember.
