Abstract

The underlying cause of death is a record that contains a sequence of events that led to death. Quarterly data reports (March-May) in 2022 at Fatimah Hospital, inpatients who died were 44 patients. Fatimah Hospital since the form was created in 2012 has never been revised. The purpose of this study was to redesign the cause of death form at by identifying according to Huffman's theory three aspects of the form, namely physical aspects, anatomical aspects, content aspects, and identifying user needs on the cause of death form. The data collection instruments used were observation, brainstorming, documentation, and interviews with the five main informants in the hospital. The five main informants consisted of one doctor on duty/doctor in charge of patients, two nurses, one head of medical records, and one medical record officer. The results obtained on the physical aspect, used HVS A4 70 gram paper, white color, rectangular portrait, using black ink on information items, and color ink for logo and hospital names. Anatomical aspects, a heading that includes logo, hospital identity, form page number, and label. Body parts according to Ms. software. Word and close for form authentication. Aspects of content, according to WHO standards in ICD-10 Vol 2, information on time intervals, types of death, and further details on types of death. This research is expected to be a consideration for the design of the proposed cause of death form at Fatimah Hospital, can be used as a basis for designing the cause of death form, and for further researchers, can be more detailed including further information on the type of patient death
