Abstract

Retention is the process of separating or moving between inactive medical record documents and active medical record documents in the storage (filling) room. The lack of retention and destruction will have an impact on the full shelf storage of medical record files. There are 700 inactive medical record files that should have been destroyed.This study aims to analyze the causes of the unfulfilled retention and destruction of medical record files at the Puskesmas Kedungwungu Banyuwangi based on Amstrong and Baron's performance theory, namely personal factors, leadership factors, team factors, system factors, and situational factors. This type of research is qualitative research. Data collection techniques use interviews, observation, documentation, and brainstorming. The results of the study are based on personal factors, namely officer education that is still not in accordance with applicable standards, training that has not been obtained by officers, leadership factors, namely leadership factors that have not run well, team factors, namely colleagues who support each other and work together, system factors, namely there are no facilities and SPO that help for the implementation of retention and destruction activities. The efforts to correct the problem are proposing to add medical record officers, making retention and destruction SPO, and conducting training on medical records.
