Abstract
Filing room is one of the most suppot rooms in medical record services, medical record files for inpatiens and outpatient kept safely because its secret and a legal aspect. In the storage of medical record file in Kademangan Public Health Service, there were misfile ( loss and misplacement of medical record) of 366 outpatient medical record file in a week, there was 41 misfile or missing medical record files. The impact of incident misfile are delayed in process patient care, duplication on medical record number, not continuous medical record content (Anggraeni, 2013). The aims of the study is do a primary analysis of factors causing misfile occurrence in filing medical record unit Kademangan Primary Health Care Bondowoso. This study uses qualitative methods and data acquisition in this study is through in interviews, observation and USG ( Urgency, Seriousness, Growth). Result of this research, get priority problem of cause of missfile event that is motivation from head of medical record. Alternative solution is head of medical record give more motivation to clerk of medical record especially in filing so it expected to create discipline of filing officersin do their work. The creating discipline of filing officers when they workcan make activitie of management medical record become maximum. Keyword : Public health service, Misfile medical record file, USG
