Abstract
Medical record is a file that contains records and documents about patient identity, examination, treatment, actions and other services that have been provided to patients. Medical records must be returned to the medical record unit after completion of service. The deadline for returning medical records is no later than 2x24 hours after the patient is declared out of the health care facility. If the return of the medical record is more than 2x24 hours, it can be declared late. The impact that occurs due to delays in returning medical records is that it can affect the waiting time of patients to get services, besides that it can also hinder further activities such as assembling and coding and being late in making reports. Based on a preliminary study in December 2021, researchers obtained information on the delay in returning inpatient medical records, the delay in returning inpatient medical records at the Jambesari Health Center from October to December 2021 for the timeliness of return, which was 13.7%, while for the delay in return, it was equal to 13.7%. 86.3%. The purpose of this study was to analyze the factors causing the delay in returning inpatient medical records. This type of research uses qualitative research, the subject is 1 head of the puskesmas, 3 nurses, 1 medical recorder, the object of this research is inpatient medical records. The results found in this study are that the first priority is that there is no SOP (Standard Operational Procedure) regarding the return of inpatient medical records so that an effort is determined to make an SOP for returning inpatient medical records in accordance with medical record service standards.
