Abstract

Medical records are confidential, because of the confidentiality of medical record, borrowing and returning them must be structured and systematic to reduce the loss of medical record files and leaking of medical record contents to irresponsible parties. Confidentiality of medical record files can be done by good storage. The implementation of storing medical record files at the Puskesmas Jember Kidul still uses a manual system. The problem that occurs is that medical record files are often found that are misplaced or missing. The purpose of this study is to design and create a medical record file storage system at the Puskesmas Jember Kidul which is expected to facilitate the work of officers. The method used in this research is the prototype method. The collection of data through the interviews, observation and brainstorming. The results of the research is a requirement system of functional and non functional information system retention and extermination of medical record to be made. Then the necessity is transformed into a form of system design that is system flowchart, context diagram, data flow diagram and entity relationship diagram. The design transform into machine language using PHP and MySql database. The test results show that the information system made in accordance with the necessity and there are no errors. The existence of an information system can facilitate the implementation of lending andreturn of medical record files and improve data storage security.
