Abstract

Balung Regional Hospital has patient and family education forms. The form is not in accordance with SNARS 2018 edition 1.1. In order to meet the needs of the 2018 SNARS hospital accreditation verification survey, edition 1.1, it is necessary to organize an integrated medical record form that meets the standards. The standard is based on SNARS 2018 edition 1.1 in chapter 4 of the PP2 standard, it is explained that there is a procedure to integrate and coordinate the care provided to each patient. This study aims to create a patient and family education form. This type of research is qualitative. Data collection is done by means of observation, interviews and brainstorming. The theory used in this study is Huffman (1999) which includes physical aspects, anatomical aspects and content aspects. The results of this study are in the form of patient and family education forms. Based on the physical aspect, the form uses 70 gram A4 paper, with a landscape shape, using black ink and white paper. Based on the anatomical aspect, the head of the form (Heading) is located at the top, there is no Instruction, uses the Times New Roman 12 font, and how to fill out the form manually. Based on the content aspect of the form, the items are appropriate, only a few items need to be removed and they have been arranged sequentially and on the form the terms, abbreviations, and symbols used are in accordance with hospital standards. Suggestions related to the integrated patient education form the home side evaluates according to needs related to physical aspects, anatomical aspects and content aspects. A review of the completeness of filling by the hospital must also be carried out periodically.
