Abstract

Sukabumi Health Center (Puskesmas) of Probolinggo City was a non-inpatient health center that has never carried out retention and destruction of medical record files since the implementation of the medical record file storage system. Sukabumi Health Center (Puskesmas) of Probolinggo City was a non-inpatient health center that has never carried out retention and destruction of medical record files since the implementation of the medical record file storage system. This study was using qualitative research. The data collected using in-depth interviews, observation, and documentation. The results showed that the man factor was the latest education of officers in accordance with medical record professional standards, lack of knowledge of officers regarding the implementation of retention and destruction activities, and officers who had never attended training on retention and destruction of medical record files. The money factor was the absence of a budget plan for retention and destruction of medical record files. Material factors were the absence of archive retention schedules and documentation of destruction and the absence of special shelves and storage space for inactive medical record files. The machine factor was the availability of a medical record file enumerator but there was no special scanner for retention. The method factor was the absence of special SOPs for retention and destruction. The priority of the main problem was the method element, which was the unavailability of special SOPs for retention and destruction. The research suggestion was the need to make a special SOP for file retention and destruction and be socialized to medical record officers.
