Abstract

It is very important to record data about the actions taken by medical personnel against patients to be documented in the informed consent form. So that the informed consent form needs to be designed in such a way as to meet user needs. Based on the preliminary survey at the Candipuro Community Health Center the consent form for mental illness patients still uses the usual outpatient form, it does not reflect the mental disorder patient's informed consent form. The purpose of this study was to design an informed consent form for mental patients at the Candipuro Health Center Mental Health Service Unit. The method used was qualitative with data collection techniques of observation, interviews, and brainstorming, the research subjects were 1 medical record officer, 2 mental patient nurses, a doctor who handled mental patients, and the head of the puskesmas. Data analysis in this study used data reduction analysis, display data, and conclusions. The results of this study are the design of an informed consent form for mental patients according to user needs based on physical aspects using HVS F4 paper with a weight of 70 grams, anatomical aspects adjusting form design standards, and aspects of form content adjust to user needs by including administrative data and clinical data. consists of diagnosis and the basis for its enforcement, medical action consisting of indications, procedures, goals and risks, as well as other information required in the informed consent. The design of a new informed consent form for mental patients can be applied at the Mental Health Service Unit of the Candipuro Community Health Center to support the completeness of information on the medical records of patients with mental disorders and to avoid malpractice.
