Abstract

Incompleteness Patient Identification Filling on medical records form is important to decide the ownership of that form. Incompleteness filling of patient identification in Pandanwangi Public Health Center Malang impact on the low quality of medical records because medical records does not contain complete information. Based on the preliminary of incompleteness filling identification patient based on 30 medical record documents found thecompleteness of filling in patient identification was not 100%. This research is to identify causative factor of incompleteness patient identification filling on the medical record document in Pandanwangi Public Health Center Malang based on 7M management element that are, manpower, machines, materials, method, motivation, media and money. The type of this research qualitative with observation, interview, documentation and brainstorming. The result of research based on manpower is never been any special training regarding of medical records, machine variable there are office stationary, motivation variable is the using of reward and punishment, method variable is there is no Standard Operasional Procedure and there was no monitoring and evaluating process, material variable is the identification form alredy effective and efficient, media variable is the environment alredy good enough, and the money variable is there is no budget for meical record training. The conlusion of rhe research of the incompleteness factors of patient identification are manpower, method and money variable. The suggestion to repair the problems are the public health center is to provide training and seminars for officers, monitor and evaluate incomplete patient identification filling regulary, make Standard Operasional Procedure and do socialization of the Standard Operasional Procedure, and make budget for training the officers.
